**This position is located at the Palm Desert VA Clinic** This position will exemplify the LLUHS Mission, Vision and Values to support and implement the Core Pillars of the Strategic Plan: World Class Distinction, Quality & Service Excellence, Teamwork & Synergy, Partnerships, and Leadership & Stewardship. The Care Manager-RN 2 is responsible for the management of patient care in the assigned LLUHC/VA Community Based Outpatient Clinic (CBOC) Patient Aligned Care Team (PACT) within the RN Scope of Practice. This includes coordination of assigned high risk patient caseload in order to expedite patient movement throughout the health care setting and facilitating continuity of care. Evaluates daily physician assigned patients in the CBOCs, those transitioning from in-patient to ambulatory care status and those receiving home care support, in collaborating with the Home Health Referral Service, Managed Care or In-Patient Case Managers and Social Work. Serves as a consultant and resource to patients and other members of the health care team. Works closely with VA Liaison, CBOC Medical Director, and CBOC Clinic Director. Performs other duties as needed.
Current and unrestricted California RN license required. Current BLS/CPR certification required. Associate of Science degree in Nursing or RN diploma required. Bachelor's degree preferred. Minimum two years of RN experience in a clinical setting, including patient triage and assessment, required. Able to read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer and software programs necessary to the position; troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place/patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch. Flexible to adjust work schedule, and travel to other VA clinics, as needed.
